Zing Health lists HMO, PPO and Chronic Condition Special Needs Plan options. Those labels describe different plan arrangements; they do not establish that every option is available in your county or appropriate for you.

Identify the plan before comparing benefits
Use the current plan name, contract and plan identifiers, service area and year. A prior-year advertisement cannot confirm today's premium, network or supplemental allowance.
| Plan feature | Question to resolve |
|---|---|
| HMO | Which network, referrals and out-of-network exceptions apply? |
| PPO | How do covered out-of-network services and costs work? |
| C-SNP | Does the plan accept your qualifying condition, and what verification is required? |
| Extra benefits | What limits and eligibility conditions apply to this exact plan? |
A C-SNP requires more than an interest in extra benefits
Medicare requires Part A and Part B, residence in the service area and the plan's special eligibility criteria. The plan verifies the qualifying condition. A C-SNP is different from a D-SNP for people who meet Medicare and Medicaid requirements.
Use the carrier's member resources
Zing's official site links provider searches and plan documents. Check the actual physician, hospital and office address. Confirm whether dental, vision or hearing services use a separate provider arrangement.
- Read the Summary of Benefits and Evidence of Coverage.
- Compare medical copayments and annual out-of-pocket limits.
- Ask about referrals and authorization.
- Check the current enrollment period and coverage start date.
Keep the comparison neutral
No carrier name establishes that a plan is the best choice for everyone. Compare access, costs and rules with other available coverage. Any quoted allowance or reduced cost sharing should be confirmed in your own plan documents, including any conditions on its use.
