UCare Medicare Plan Closures: Check Your Current Coverage

UCare Medicare plans closed after 2025. Distinguish those closures from the Medicaid transition and verify current benefits, claims and enrollment rights.

UCare reports that its Medicare Advantage, Medicare Supplement, MSHO and Connect + Medicare plans closed on December 31, 2025. Old articles recommending enrollment in those products should not be used to choose current coverage.

The Medicaid transition is a different issue

UCare's current announcement discusses a transition of certain Medicaid plans to Medica One Health Plan on October 1, 2026. That does not mean a former UCare Medicare Advantage policy automatically continues under a new name. Identify both programs separately if you have Medicare and Medicaid.

Record What to verify
Medicare enrollment Your current plan or Original Medicare status.
Medicaid enrollment Your current state coverage and managed-care plan.
Provider arrangements Which current network and authorization apply.
Earlier claims Coverage in effect when care occurred.

Check dates before relying on transition guidance

Some closure notices describe special enrollment or Medigap rights with early-2026 deadlines. Those historical dates are not open-ended permissions to change coverage now. Medicare and your state counseling service can help identify a current enrollment opportunity.

Keep care and billing organized

  • Give your doctor's office the current insurance card.
  • Ask how existing appointments and authorizations will be handled.
  • Retain closure notices and claim explanations.
  • Confirm the effective date before assuming a replacement plan is active.

Where to get help

UCare's official product-change page lists former-member resources. Minnesota Aging Pathways provides Medicare counseling at 1-800-333-2433. People using disability-related services can also consult Disability HUB at 1-866-333-2466.

Ask about the exact program named in your notice. A similar company name, an old benefit card or a general transition announcement cannot establish your personal coverage.
