Commonwealth Care Alliance's Massachusetts programs should not be described as one general Medicare Advantage offer. CCA states that its Massachusetts Medicare Advantage PPO plans ended January 1, 2025. It also records One Care's transition from a Medicare-Medicaid Plan to a D-SNP on January 1, 2026.

Identify the actual program
| Program or record | How to interpret it |
|---|---|
| Closed Massachusetts PPO | Historical documents for past coverage. |
| One Care | Current program-specific Medicare and Medicaid rules. |
| Senior Care Options | Separate age and program eligibility requirements. |
| CCA in another state | Use that state's product and materials. |
Do not apply a Massachusetts program description to another state or assume every person with Medicare qualifies. Verify age, residence, Medicare and MassHealth status, plus any other enrollment conditions, with the responsible program.
Understand coordinated coverage
Ask how the care team handles Medicare services, Medicaid services and community support. Confirm current doctors, facilities and authorized representatives. Coordination can help organize care, but it does not mean that every requested service is covered without a decision.
Keep the correct documents
- Current member card and enrollment notice.
- Product-specific Evidence of Coverage.
- Provider directory and annual changes.
- State eligibility notices and important decisions.
A Medicaid eligibility change can affect a special-needs arrangement. Report changes promptly and ask what notices or continuation rules apply instead of assuming membership continues unchanged.
Claims and assistance
For an older PPO bill, use the documents governing that date of service. For a current problem, follow the appropriate benefit's coverage and appeal process. CCA offers language assistance, while Massachusetts SHINE and the state program can help clarify Medicare and MassHealth responsibilities. A historical closure does not by itself create an enrollment exception today.
