Massachusetts residents use federal Medicare, but the state's Medigap structure differs from the lettered plans used in most states. Comparing a national Plan G advertisement directly with a Massachusetts supplement can be misleading.

Understand the two main arrangements
Original Medicare pays covered Part A and Part B services. A Massachusetts Medigap policy can help with specified remaining costs. Medicare Advantage is a private-plan alternative for receiving Medicare benefits, with its own cost sharing and network arrangements.
Massachusetts supplement names
| Policy type | Important distinction |
|---|---|
| Core | Provides the state's basic standardized supplement benefits. |
| Supplement 1A | Includes additional specified benefits but not the Part B deductible. |
| Supplement 1 | Includes Part B deductible coverage; restricted to people eligible for Medicare before 2020. |
Compare the official Massachusetts chart and the insurer's outline for hospital deductibles, skilled nursing coinsurance and other differences. A supplement does not make every Medicare-excluded service covered, and it is not used alongside Medicare Advantage.
Check access and costs
With Medicare Advantage, verify your medical group, hospital and specialists under the exact contract. With Original Medicare, confirm that the provider accepts Medicare and review how the supplement handles covered cost sharing. Monthly premiums are only one part of the yearly budget.
MassHealth is a separate program. Eligibility for full benefits or a Medicare Savings Program needs its own review; being enrolled in Medicare does not automatically establish all MassHealth benefits.
Use SHINE before making a change
The Massachusetts SHINE program provides independent Medicare information and counseling for residents and caregivers. Ask for help reviewing options, state supplement rules or financial-assistance programs. Private agents and SHINE counselors serve different roles.
Keep the current coverage documents, Medicare effective dates and provider list together. Confirm the new plan's start date and any effect on retiree coverage before cancelling an existing policy. The Medicare annual plan election period and supplement purchase protections are not interchangeable.
