There is no form that makes every eyeglass purchase reimbursable by Medicare. First confirm that the eyewear qualifies for a covered benefit and that the supplier participates in Medicare. Then determine who should submit the claim.

Check coverage before paperwork
Part B covers a limited eyewear benefit after qualifying cataract surgery with an intraocular lens. Routine glasses are generally excluded. Upgraded frames and other additions can remain your responsibility even when the standard item is covered.
| Your coverage | Where to start |
|---|---|
| Original Medicare | Ask the participating supplier to submit the covered-item claim. |
| Medicare Advantage vision benefit | Use the plan's supplier and reimbursement instructions. |
| Separate vision insurance | Use that insurer's own claim process. |
If you must submit an Original Medicare claim
This is generally uncommon because providers and suppliers normally file covered claims. If necessary, Medicare provides the Patient Request for Medical Payment form, CMS-1490S, with instructions.
- Complete the current official form.
- Include an itemized bill.
- Explain why you are filing rather than the supplier.
- Attach relevant supporting documents.
- Use the appropriate address specified in the instructions.
Claims generally must be filed within 12 months of the service date unless an exception applies. Keep copies and follow the instructions for the type of item supplied.
Track the result
Review your secure Medicare account or Medicare Summary Notice. If payment is denied, the reason may involve the benefit, supplier, documentation or timing. Follow the notice's review procedure rather than repeatedly submitting the same bill.
Use verified secure channels for your Medicare number. A retail receipt or completed claim form is not a promise of reimbursement.
