Locate the current document
Use Wellcare’s official plan-information page to select the correct state, plan and year, then open its pharmacy resources. Existing members can request documents through member services.
Match the plan name and identification number, not just the company logo. An archived 2022, 2024 or 2025 document does not establish coverage for 2026 or 2027. If reviewing an old claim, use the document effective on that service date.

Read the Annual Notice of Change together with the new Evidence of Coverage and updated list. Compare the complete yearly insurance cost, including premiums, deductible and expected cost sharing. Do not judge a plan solely by the lowest advertised premium.
