CDPHP's Medicare Advantage options must be compared by plan and service area. Its 2026 individual materials list Core and several Rx HMO products. These names do not describe identical benefits or guarantee availability throughout New York.

Core and Rx plans are different
CDPHP identifies its 2026 Core HMO as a plan without included Part D coverage. Other listed products include Rx coverage. That distinction matters before enrollment: do not assume a medical-only HMO can simply be combined with a separate Part D plan.
Joining standalone Part D while enrolled in most Medicare Advantage HMOs can end the Medicare Advantage enrollment. Check the exact arrangement with Medicare and the plan before submitting an application.
| Comparison item | Where to verify |
|---|---|
| Service area | Current county list and plan identifier. |
| Part D inclusion | Product name and Evidence of Coverage. |
| Doctors and hospitals | The plan-specific provider directory. |
| Additional benefits | Benefit limits and permitted uses in current documents. |
Check HMO access
CDPHP's individual HMO materials require selection of a primary care physician and use of plan providers for routine covered care, subject to applicable exceptions. Confirm your specialist and hospital rather than assuming that every office accepting CDPHP accepts this product.
Look beyond the premium
- Hospital daily charges and outpatient surgery costs.
- Medical out-of-pocket maximum and what does not count toward it.
- Therapy, advanced imaging and equipment expenses.
- Dental, vision and hearing allowances and restrictions.
- The continuing Medicare Part B premium.
Renewal and enrollment
Use 2026 documents for 2026 care and the next year's official materials when considering renewal. Keep Medicare enrollment timing separate from the date marketing begins. Retain the confirmation showing your accepted plan and effective date.
