If an old card, brochure or search result says UnitedHealthcare Medicare Advantage Assure PPO, do not use that name alone to determine today's benefits. Identify the policy before comparing it

A similar new name is not proof that benefits stayed the same. Ask the insurer whether the old policy renewed, changed or ended. Keep the written notice rather than relying on a search engine's cached description.
What PPO does and does not mean
Medicare PPO plans generally include covered out-of-network care at different cost sharing. That flexibility does not guarantee every service, every doctor or the same price outside the network. Confirm provider acceptance and whether a proposed service needs prior authorization.
Compare the medical out-of-pocket limits, hospital and specialist costs, additional plan premium and any extra-benefit limits. Continue including the Part B premium. Do not transfer figures from another UnitedHealthcare PPO to an Assure-labeled document.
If your documents no longer match
- Call the member service number on the current insurance card.
- Ask which plan is active and its effective date.
- Request the current Evidence of Coverage and provider directory.
- Check any available enrollment right before changing coverage.
An old plan name by itself is not evidence that you lost coverage or that a replacement enrollment was completed.
