Moda Health's official December 2024 provider update states that Moda and Summit Health stopped offering Medicare Advantage plans in Oregon starting January 1, 2025. Older Summit brochures should not be used as offers for current enrollment or as proof of present benefits.

Which Summit Health do you mean?
The former Oregon insurance offering is different from the Summit Health medical group whose offices publish accepted-insurance information. A practice accepting certain Medicare plans does not mean it issues the old Oregon Summit policy.
| Question | Where to verify |
|---|---|
| What is my active insurance? | Current insurer, Medicare account and enrollment records. |
| Who handles an old claim? | Administrator identified in documents for the service date. |
| Does a Summit practice accept my plan? | The exact practice and current insurer. |
| What can I enroll in now? | Current Medicare Plan Finder and applicable enrollment rules. |
If you still have an old card
Confirm which coverage is active before a planned appointment. Give the provider current insurance details. For a bill from an earlier coverage period, retain the service date, explanation of benefits and notice. The insurer responsible for that date may differ from your current insurer.
Review current choices separately
A historical plan-ending enrollment period is not automatically available today. Ask Medicare or SHIP which enrollment rights apply to your current circumstances. If considering Medigap, confirm guaranteed-issue or other enrollment protections and their deadlines before making a change.
- Find plans for your present home address and year.
- Verify doctors and facilities.
- Compare premiums and expected service costs.
- Check consequences for other coverage.
- Keep confirmation of the chosen plan and start date.
No benefit amount from an old Summit marketing article should replace this current verification.
