Medicare can cover qualifying ambulatory heart-rhythm monitoring when ordered to evaluate a medical problem. A Holter monitor is part of a diagnostic service; it is different from purchasing a consumer wearable for general fitness tracking.

Confirm the ordered monitoring service
Your practitioner should identify the reason for testing and the appropriate duration and method. Holter recording, longer event monitoring and implanted monitoring are not automatically the same benefit or billing service.
| Detail | What to ask |
|---|---|
| Order | Has the monitoring service received it? |
| Medical reason | Is the diagnostic need documented? |
| Duration | What recording period did the clinician request? |
| Interpretation | Who will review the recording and communicate results? |
Coverage is not based on the device alone
The national electrocardiographic policy distinguishes covered diagnostic uses from excluded arrangements. Home testing requires documentation of medical necessity. A seller's statement that a monitor measures heart activity does not establish Medicare coverage.
A diagnostic monitor should not be confused with the limited screening EKG associated with the Welcome to Medicare visit. The reason and rules for each service differ.
Review costs and practical arrangements
- Ask whether fitting, recording and interpretation are billed separately.
- Check assignment and any deductible or coinsurance.
- Confirm the approved provider and authorization under Medicare Advantage.
- Clarify device return instructions and any noncovered charges.
For covered diagnostic tests in an office or independent testing facility, the usual Part B deductible and 20% cost sharing generally apply. Hospital settings can involve additional charges.
After testing
Contact the ordering practice about results. If the bill differs from the estimate, request an itemized explanation and review any denial notice. Follow your clinician's instructions about symptoms; do not assume every monitor provides live emergency observation.
