Original Medicare generally does not pay for an adult day program whose purpose is ongoing supervision, social activities or nonmedical personal assistance. A center's name or the presence of health workers does not automatically make the entire daily fee a Medicare benefit.

Identify what the fee includes
| Charge | Coverage question |
|---|---|
| Attendance, meals or supervision | Is this nonmedical long-term support? |
| A specific medical service | Does it independently meet Medicare's coverage and billing rules? |
| Transportation | Which program, if any, authorizes and pays for it? |
| PACE services | Are you enrolled and is the service arranged by your PACE team? |
PACE is a separate program
The Program of All-inclusive Care for the Elderly can include adult day primary care and coordinated support. Eligibility requires being at least 55, living in a participating service area, meeting the state's nursing-home-level-of-care standard and being able to live safely in the community with PACE assistance.
PACE is not automatic adult day coverage for every Medicare member. Discuss enrollment, service arrangements and potential premiums with the program before changing existing coverage.
Home health and day attendance
Attending adult day care does not automatically prevent qualifying Medicare home health services. However, eligibility for home health does not make the day program's general attendance fee covered. Ask the ordering practice and agency to explain the two arrangements separately.
Before choosing a center
- Request an itemized fee schedule and cancellation policy.
- Ask your state Medicaid office about applicable community programs.
- Confirm any Medicare Advantage supplemental benefit in your own plan.
- Check transportation, accessibility and emergency procedures.
Keep written authorization and cost information. If the center says a service is covered, ask which benefit and payer it will bill rather than relying on a general assurance.
