An Aetna Medicare Advantage appeal asks the plan to reconsider an adverse coverage or payment decision. Start with the written notice. The correct form depends on your product and whether you are disputing authorization, payment or a fast-ending-care decision.

Choose the correct route
| Situation | Starting point |
|---|---|
| Medical authorization denied | Aetna's medical authorization appeal process. |
| Claim payment disputed | The member claim-denial appeal route. |
| Hospital discharge or covered care ending | The fast-review instructions and QIO named in the notice. |
| Service complaint without a coverage dispute | The grievance process. |
Prepare the information
- Record the notice date, service, decision and reference number.
- Explain what you disagree with and the outcome requested.
- Attach relevant clinical or billing evidence.
- Include representative authorization when required.
- Keep a copy and proof of submission.
Aetna's official page lets members select the applicable state, county and plan and distinguishes authorization appeals from claim appeals. Do not substitute a provider's contractual billing-dispute form for a member's coverage appeal.
Watch the deadline
Medicare generally allows 65 days from the date on the initial Medicare Advantage denial notice to request reconsideration. Fast hospital-discharge or ending-care reviews use much shorter deadlines. Follow the actual notice immediately; do not apply the routine filing period to an urgent discharge.
When speed matters
If waiting for a standard service decision could seriously jeopardize life, health or the ability to regain maximum function, ask the plan about expedited review and obtain your clinician's support. A payment dispute after care is not automatically eligible for a fast decision.
After submission
Confirm receipt and track the case number. Read the written result and further appeal instructions. Use verified insurer channels for private documents; the general contact form on this website is not Aetna's appeal portal.
