An AARP Medicare Advantage Choice Plan 2 PPO name in an old article is not a current benefit guarantee. UnitedHealthcare plan names, service areas and benefits must be matched to a specific year and identifier. Do not assume that every product called Choice Plan 2 has the same network or costs.

Start with an identity check
Use the insurer's current plan search or Medicare Plan Finder for your home address. Compare the complete plan name and identifier with your brochure or renewal notice. Determine whether the policy is an individual plan or an employer-sponsored retiree option before applying general enrollment information.
| Evidence | What it establishes |
|---|---|
| Current plan listing | Availability for the selected place and year. |
| Summary of Benefits | Premiums and major service costs. |
| Provider directory | Listed network participation. |
| Evidence of Coverage | Full conditions, limits and authorization rules. |
Do not interpret PPO as unlimited coverage
A PPO may cover out-of-network services, but the provider must agree to treat you and the service must meet coverage rules. Confirm costs before planned care. Emergency protections are different from permission to schedule any service anywhere at network prices.
Ask about both in-network and combined medical limits, if applicable. Check additional benefits individually. An allowance may require participating providers and have an annual cap. Include the Part B premium and any plan premium when estimating costs.
For an existing member
Read the Annual Notice of Change and any nonrenewal notice. Ask the insurer which policy will be active next year rather than assuming a matching name means unchanged coverage. Check any available enrollment right, then keep written confirmation of the effective date. This guide does not assert that a particular Choice Plan 2 remains open for enrollment.
