The Medicare Prescription Payment Plan is a voluntary billing option for people with Part D, including Part D inside a Medicare Advantage plan. It spreads covered out-of-pocket Part D costs across the calendar year. It is not a new insurance policy, a discount or financial assistance.

Discuss your budget with the plan
Ask the plan how it will calculate your monthly bill, when payments are due and what happens if costs change during the year. Do not assume every monthly installment will be identical. Request an estimate based on your situation before deciding whether this billing arrangement is useful.
This option does not remove coverage requirements, network rules or the need for applicable approval. An expense that is not a covered Part D benefit does not become covered because you selected monthly billing.
Keep annual limits and assistance separate
The annual Part D out-of-pocket threshold is $2,100 in 2026 and $2,400 in 2027 for counted covered spending. Premiums and noncovered purchases are not included in that protection. The payment option changes when you pay applicable expenses, not which expenses count.
If affordability is the problem, ask about Extra Help or other assistance for which you may qualify. A payment arrangement and a program that reduces your costs solve different problems.
