Panniculectomy After Weight Loss: Medicare Coverage Requirements

Weight loss does not automatically qualify skin removal for Medicare coverage. Separate cosmetic contouring from documented medical or functional problems.

Medicare does not automatically cover skin-removal or body-contouring surgery after weight loss or gastric bypass. A panniculectomy may qualify when it meets the applicable medical-necessity policy. Surgery performed only to change appearance is generally excluded.

Document the actual problem

Ask the surgeon to identify the condition, functional effects and proposed operation. A large weight change or a history of bariatric surgery is not, by itself, a coverage approval.

Record Why it matters
Skin findings and symptoms Distinguish a medical problem from appearance alone.
Prior care and response Show whether required management was attempted.
Functional limitations Document the effect on daily activities.
Weight and surgery history Support any timing or stability requirements.

Use the policy that applies to you

For example, LCD L39051 discusses the extent of the overhanging abdominal tissue, documented recurring or persistent skin problems, prior care and functional impairment. Its full criteria and post-weight-loss conditions must be reviewed by the treating practice.

Separate covered work from cosmetic additions

Panniculectomy, an abdominoplasty, liposuction and other body-contouring procedures are not automatically equivalent. Ask for an itemized plan that identifies any private-pay components. Approval of one medically necessary procedure does not establish payment for the whole package.

Before scheduling

  • Confirm the applicable policy and documentation.
  • Check surgeon, facility and authorization requirements.
  • Ask about anesthesia, follow-up and your cost sharing.
  • Keep the written decision and estimate.
