When Medicare Advantage Denies a Service: Reasons and Next Steps

Understand coverage determinations, written denial reasons, appeal deadlines and urgent review without assuming every denial is correct.

A Medicare Advantage plan can deny a particular service or payment request, but it must follow Medicare coverage and appeal requirements. A denial is a decision that can be reviewed; it is not proof that you have no further options.

Find out exactly what was denied

The issue might involve medical necessity, missing records, a service exclusion or an applicable network or authorization rule. Request the written decision and the criteria used. An unprocessed claim, billing mistake and formal coverage denial may require different first steps.

Ask for a coverage determination

You, your representative or your provider can request an organization determination for a service. If the answer is unfavorable, the notice explains how to request reconsideration. A general member-service conversation may not start that appeal.

Watch the deadline and urgency

A Medicare Advantage medical appeal generally must be filed within 65 days from the initial denial notice. Explain why if you request review after the deadline. Follow the notice for the correct destination and required details.

For eligible urgent cases, an expedited appeal can require a decision within 72 hours. Ask your provider to explain why a standard wait could seriously jeopardize your health or recovery. Payment-only disagreements do not automatically qualify for urgent review.

If the plan upholds the decision

Read the next notice for independent review and later appeal rights. A complaint about service quality is separate from an appeal of coverage. Do not assume you owe the entire bill simply because a claim was denied; request an explanation of your responsibility and any applicable protections.
