Medicare and prostate cancer: tests, procedures and coverage review

Medicare can cover eligible medical services related to prostate cancer, but screening, diagnostic tests and treatment are different benefit questions. A covered PSA screening does not automatically approve every later imaging study, procedure or treatment approach.

Start with the purpose of each service

Service category What to confirm
Preventive screening Age and timing rules for the screening benefit.
Diagnostic evaluation Symptoms, findings and the ordered test's medical necessity.
Surgery or radiation Exact procedure, indication and applicable coverage policy.

A diagnosis is not a technique-specific approval

Policies for prostate surgery, proton radiation, specialized imaging and other approaches may have different requirements. Ask the treating team to identify the proposed service precisely and which current Medicare contractor or plan rules apply.

Do not assume that a commercial insurer's policy or a center's advertisement establishes Original Medicare coverage. A decision about one technique should not be reused as approval for a different one.

Plan the insurance steps with the office

  1. Verify the specialist, hospital and testing facilities.
  2. Check referral and authorization requirements.
  3. Request written coverage information when available.
  4. Separate facility, physician, imaging and pathology costs.
  5. Keep records and appeal instructions.

For Medicare Advantage, use your exact plan's documents and member service channel. For Original Medicare, ask about assignment and expected deductible or coinsurance.
