Identify the exact plan

Write down the plan name, contract and plan identifier, county and coverage year. Similar names such as “Choice” can refer to different products. An employer or union group plan may have different terms from an individual plan advertised to the public.

Plan feature What to compare
HMO or HMO-POS Routine network rules and any specifically permitted point-of-service care.
PPO Network costs, outside-network charges and whether the provider agrees to see you.
Special Needs Plan Required eligibility and coordination with any other benefits.
Extra benefits Personal eligibility, limits and separate provider requirements.

Check medical access and total costs

Confirm doctors and hospitals with the plan and the provider’s office using the specific plan ID. Ask about referrals and prior authorization for anticipated care. Emergency and urgently needed care have protections; routine out-of-network appointments follow different rules.

Compare plan premiums, deductibles, hospital and specialist charges, and the annual medical out-of-pocket limit. You generally continue to owe the Part B premium. A low premium or spending-card benefit is not a complete cost comparison.

Use current member documents

Start with official member resources for directories and plan documents. Read the Evidence of Coverage and, for renewal, the Annual Notice of Change. Verify the year before using a saved PDF.

  1. List the services and providers that matter to you.
  2. Compare written benefits across the available options.
  3. Check the enrollment opportunity and requested effective date.
  4. Keep the confirmation and current card.
