IRMAA 2026: Medicare Part B Income Brackets and Review Options

Check official 2026 Part B premium brackets, the income year used by Social Security and how qualifying life changes can support a review.

IRMAA is an income-related adjustment to Medicare Part B and Part D premiums. It is separate from a late-enrollment penalty. For 2026, Social Security generally uses your 2024 tax information, not simply your current paycheck.

These are full Part B premiums per person, before any separate penalty. Do not add the standard premium again. Married people who filed separately and lived together during the tax year use a different table: above $109,000 but below $391,000, the total is $649.20; at $391,000 or more, $689.90.

What income is counted?

For this calculation, MAGI is adjusted gross income plus tax-exempt interest. Social Security normally uses the most recent IRS information, generally from two years earlier. If older information was used or a return was amended, contact Social Security with supporting records.

The amount withdrawn from a bank account is not necessarily taxable income. Taxable gains, retirement distributions and other items can affect adjusted gross income differently. Confirm tax treatment with a qualified tax professional rather than assuming every withdrawal counts equally.

If your income has fallen

A qualifying life-changing event, such as stopping work, reduced work hours, divorce or a spouse's death, can support a request for a new decision. Form SSA-44 requests the event and income evidence. A reduction is not automatic whenever income changes.

If you disagree with a determination, follow the appeal instructions in your notice. Correcting tax information, requesting a new decision after a qualifying event and appealing an incorrect decision are different processes.

Keep other costs separate

Part D can have its own IRMAA in addition to the plan premium. It is paid through the federal billing or benefit-deduction process, not added to the private insurer's premium payment. Medigap and Medicare Advantage premiums are also separate from this Part B table. Use your official notice for your individual amount.
